A skilled nurse is necessary when a client requires intravenous therapy, wound care and treatment, pre/post operative care, Diabetic care, catheter insertion and management, teaching and and counseling. Your registered nurse (RN) will assess, recommend and closely monitor the administration of your care while remaining in constant contact with your primary care physician to coordinate all aspects of your care plan throughout your episode. This is why we refer to our RNs as case managers. Although your Licensed Vocational Nurse (LVN) or Certified Home Health Aide (CHHA) will most likely attend to you more frequently, they report the status of condition to the registered nurse after every visit.
Your LVN will work with you in your home 1-3 times per week depending on your frequency in the plan of care that was established by your RN. Your LVN will work continuously with your registered nurse to continuously improve your plan of care. The general duties of your LVN will include:
- Implements plan of care initiated by RN
- Provide direct patient care
- Provides continual education regarding your condition
- oversees the acquisition and use of medical equipment and supplies
Frequently, the CHHA becomes one of the central aspects of the client’s path to recovery. This is because their tasks are relevant to your everyday needs. They can assist with:
- Bathing
- Eating
- Dressing
- Toileting
- Ambulation
Your RN may also recommend additional services based on your needs such as physical, occupational and speech therapy, medical social work and psychology. The goal is to make sure that you are covered every step of the way.
