At Forever Caring our home health care teams work side by side with your physician to coordinate all aspects of your care plan and deliver advanced procedures for disease management, wound care, post operative recovery, and pain management. When faced with chronic illness customized support will make all the difference.  

We communicate with your doctor regarding changes in medication frequency and dosages as well as changes in your condition from day to day to ensure that any health concern is immediately attended to. We also educate you and your family, about how to manage your condition and ensure that all medical supplies are available to you in the home. Therapy services are also provided to help you recover and get you back on your feet again.

Visiting Physicians

For those unable to leave their home due to serious illness or injury, receiving care from a medical professional at home is an invaluable resource.

A skilled nurse is necessary when a client requires intravenous therapy, wound care and treatment, pre/post operative care, Diabetic care, catheter insertion and management, teaching and and counseling. Your registered nurse (RN) will assess, recommend and closely monitor the administration of your care while remaining in constant contact with your primary care physician to coordinate all aspects of your care plan throughout your episode. This is why we refer to our RNs as case managers. Although your Licensed Vocational Nurse (LVN) or Certified Home Health Aide (CHHA) will most likely attend to you more frequently, they report the status of condition to the registered nurse after every visit.

Your LVN will work with you in your home 1-3 times per week depending on your frequency in the plan of care that was established by your RN. Your LVN will work continuously with your registered nurse to continuously improve your plan of care. The general duties of your LVN will include:

  • Implements plan of care initiated by RN
  • Provide direct patient care
  • Provides continual education regarding your condition
  • oversees the acquisition and use of medical equipment and supplies

Frequently, the CHHA becomes one of the central aspects of the client’s path to recovery. This is because their tasks are relevant to your everyday needs. They can assist with:

  • Bathing
  • Eating
  • Dressing
  • Toileting
  • Ambulation

Your RN may also recommend additional services based on your needs such as physical, occupational and speech therapy, medical social work and psychology. The goal is to make sure that you are covered every step of the way.

Skilled Nursing

Physical Therapy

Many patients recovering from an injury or illness truly benefit from the use of physical therapy. Physical therapy has been proven to significantly increase muscle strength, mobility, gait and range of motion.

Should your nurse or physician determine that therapy is beneficial to you, a P.T. evaluation will be scheduled. After providing a complete assessment of your condition, your therapist will establish a physical therapy care plan to aid in your recovery. These treatments work together with the client’s nursing plan of care to more effectively bring about desired outcomes.

Physical therapists use a variety of treatment strategies. They may employ different types of exercise, which not only promote strength but muscle endurance as well. Some may even use less routine therapies like electrical stimulation or ultrasound technology. Whether the problem is age, injury, or disease related, physical therapy is always a viable solution.

Occupational therapy is most beneficial for patients who are preparing to return to work after recovery. By focusing on injuries that affect your performance during repetitive tasks, your occupational therapist is key to getting you back to peak performance in no time!

Your occupational therapist will perform a specialized evaluation to test muscle function, endurance, visual coordination, written communication, and verbal communication.  A systematic plan of care will work to alleviate obstacles and revise daily tasks that may be difficult for you. These activities may include, but are not limited to:

  • Dressing
  • Cooking
  • Writing
  • Driving
  • Improving memory

Occupational therapy

Speech Therapy

Often times stroke, brain injury or hearing loss, can dramatically affect your ability to communicate. Speech-language pathologists provide an invaluable service for those who are experiencing difficulties speaking or swallowing. The therapist’s approach customized to identify functional speech defects and establish a plan of care to improve patient’s function. Speech therapists can also work alongside the client’s family, friends, and/or caregivers, as necessary, in procedures that are directly related to the speech-language therapy care plan.
