Getting Started With Home Health Care At Forever Caring Is Easy!

Our goal is to make the process as convenient for you as possible. We are always happy to answer any of your questions and our operators are available 24 hours a day. Here’s how to get started:

1. It begins with receiving a request for services from you, a family member/friend or your physician.

2. In order to determine your appropriateness from home health, we then must receive an order from your physician or one of our visiting physicians to assess your condition.

3. Once the doctor’s order is received, a registered nurse is assigned as a case manager. The RN begins the treatment process by first completing a written assessment of your physical, emotional, and social needs. The evaluation includes:

  • A safety review of the home.
  • A complete physical assessment of all body systems.
  • A thorough review of all prescription & OTC medications in the home.
  • A verification of medications, treatments, and physician orders with the referring physician.

4. The appropriate clinicians will then be assigned and your care will begin. The Licensed vocational Nurse (LVN) will be your most frequent visiting nurse with visits being made 1-2 times weekly. Additional services are determined by your Registered Nurse case manager and may include: a Certified Home Health Aide (CHHA), Physical Therapist, Occupational Therapist Speech Therapist, Medical Social worker or Psychologist.
