Our intermittent care for home health care includes:

Skilled Nursing

Skilled Nursing is available for those requiring wound care and treatment; pre-op/post-op care; diabetic care; catheter management; and intravenous therapy including injections; We work with patient and caregiver to ensure a functional understanding of medication administration, disease process, and maintanence.

Registered Nursing

RN case managers are responsible for the supervision and coordination of efforts between your doctor(s) and in home care team. Coordination of care is the cornerstone of our comittment to appropriate and effective care.

Licensed Vocational Nurses

Licensed Vocational Nurses are staffed to aid in basic nursing duties such as administering medication, discussing procedures, handling medical equipment in the home, and more. They work closely with our clients, their doctors, and registered nurses to keep home health moving in the right direction.

Certified Home Health Aides

Certified Home Health Aides assist clients with basic hygiene needs, some exercise, and personal care under licensed professional supervision.

CNAs

CNAs are the primary caregivers who have the most contact with our clients. They have similar responsibilities to home health aides like hygiene and monitoring vitals, but they also work under the supervision of a licensed professional.

Our rehabilitative therapies include:

Physical Therapy

Physical Therapy is offered to clients who need to increase mobility, strength, balance, and coordination. These clients work with physical therapists to create a specialized plan for each individual that will help them see results.

Occupational Therapy

Occupational Therapy is offered to clients who need work on their fine motor skills or clients who have suffered a physical impairment. The clients who qualify work with our occupational therapists to help ease tasks in everyday life.

Speech Therapy

Speech Therapy is provided for clients with dysphagia (swallowing disorders) and people who have communication impairments. These clients work with a speech therapist for one-on-one attention to help move toward resolving issues around communication and speech

Medical Social Work

Medical Social Work is for clients in need of assistance with community resources. The goal of the client and social worker relationship is to help clients arrange for services such as meal delivery, housekeeping, and transportation.

Visiting Physicians

Visiting Physicians are available to clients who are unable to leave their homes due to serious conditions and hospice care.
